    Schema therapy with marginalized populations.


    Marginalized populations are those excluded from mainstream social, economic, cultural, or political life.*

    More specifically exclusions can apply to educational opportunities, access to health/mental health care, membership in community groups or clubs, employment opportunities, or feelings of comfort, trust, and “fitting in” to the society in which members reside.  This feeling can be described as “Minority Stress”. Groups can experience exclusion and alienation by virtue of their gender, gender identity, race, religion, education level, appearance or social status, and the list goes on.

    In this issue of the Schema Therapy Bulletin, contributors will describe the application of the Schema Therapy model with three marginalized populations: Transgender and Gender Diverse Individuals, Latinos living in the United States, and Military Veterans.

    After providing an overview of the distinction between anatomy and identity, Cesar Gonzalez outlines the predominate mental health symptoms prevalent in the transgender population, and explores their relationship to unmet core needs and specific schema development. Describing his work with Adam, a young man beginning hormone therapy to continue his transition from female to male, Cesar demonstrates Gender Affirming Schema Therapy in addressing the maladaptive schemas that arose from his gender dysphoria and his parents’ unwillingness to accept his identity as he experienced it.

    Megan Fry and Suzy Redston report on the symptoms likely to bring a soldier to a mental health professional, and describe the coping modes associated with military veterans. They note that in some cases individuals with unmet core needs may be drawn to military service. The structure and functioning of the military, the trauma of war, and the struggle to readjust to civilian life after service can all be stimuli for the manifestation and elaboration of unmet core needs, schemas, and maladaptive coping modes. The case of Ryan, a 44 year old army veteran illustrates these struggles.

    Carlos Rojas addresses the important balance between understanding and accepting cultural norms and stereotypes, and seeing each member of the culture as a unique person. Specifically he writes about the culture and values of the Latino community; it’s cultural resistance to mental health care, and it’s emphasis on family, respect machismo (a man’s position in the family), and marianismo, the role and function of women in the family). He goes on to propose that Latino culture may be characterized by particular schema profiles. This understanding can help to facilitate treatment.

    Dresses, Spiderman, and Gender Dysphoria: a Gender-Affirming Schema Therapy Approach by Cesar A. Gonzalez


    Anatomy is Not Identity

    After weeks 6 to 7 weeks into a fetus’ development, the expression of a gene induces changes that alter the trajectory of the development of a fetus’ sex characteristics and how expectant parents will eventually respond to the socially acceptable question of, “Are you having a boy or a girl?” In other words, “Are you expecting a baby with a penis or a vagina?” The moment this question is answered is when there are social rules put on the expectant parents and the unborn child. 

    These sex characteristics guide social expectations of “what boys do” and “what girls do” and impact how individuals view themselves, others, and their futures – as well as which colors or clothing they will use, which recreational activities they will pursue, what occupations they will be encouraged to follow, and even how much emotion is acceptable to express, and let people know where it's available.

    Schema Therapy and Latino Patients... A Cultural Approach by Carlos Rojas (USA)


    A primary challenge in working with patients from different cultural backgrounds is being able to use cultural generalizations appropriately without losing sight of the individual patient. To succeed in this challenge, clinicians must keep in mind that variations occur between cultural subgroups just as individuals subscribe to group norms to varying degrees. Consideration of culture is essential in the process of diagnosis and treatment of culturally diverse patients. 

    In the past decade the consideration of cultural factors has gained recognition in a variety of disciplines. In this article you will find a discussion of Latino cultural factors that are influential in the development of early maladaptive schemas (as defined within the Schema Therapy Model) and the propensity for specific schemas to develop within Latinos as a cultural group.

    Treat the Soldier and Unlock the Healthy Adult by Megan Fry and Dr. Suzy Redston (Australia)

    Generally, when a person with a military background comes for psychological treatment they have been unwell, with clinically significant symptoms for at least 12 months. There is a significantly higher 12-month prevalence of all mental illnesses compared to the general Australian population. For example, the prevalence of major depression (MDD) was 6.4% compared to 3.1% in general population (1).

    Generally military personnel will present for treatment with more than one of the following problems; trauma, unstable mood, anxiety, alienation, hyperarousal or substance use including alcohol. When working with people who have been in any kind of military, it can be very difficult to soothe maladapted modes and release the Healthy Adult.

    For the purposes of this article we shall discuss people who have been in the Army when they developed their psychological problems, however the general concepts can apply to other arms of a Defence Force. We shall aim to conceptualise the schema presented in terms of what draws people to join a Defence Force
     which can give clues to early maladapted schemas; how the training to be a soldier causes and strengthens maladapted coping modes and how these coping modes are not conducive with a healthy civilian adult. The final stage shall be a case study by one author (MF) to illustrate some of our ideas.

    Amsterdam has some of the most beautiful architecture in the world, a vibrant city inhabited by a multiracial community that celebrates a long history of inclusiveness. Today's Amsterdam is growing at an amazingly fast pace, with a lively restaurant and nightlife scene that is ever-changing. Much of Amsterdam still looks like it did in the 1600s, the Dutch Golden Age, when it was the world's wealthiest city.     CASE STUDIES IN SCHEMA THERAPY

    Issue 8 of the Schema Therapy Bulletin offers three case presentations using Schema Therapy. 

    In the first article “The Hedgehog and the Prickles,” Galit Goren Gilead presents her very creative work in helping a 12-year-old boy who refused to go to school to work with his Avoidant Coping Mode. Using play therapy techniques, without the interpretive efforts in traditional psychodynamic therapy, the author was able to bypass and then weaken the avoidant coping mode, identify and strengthen the happy child mode, and begin to develop a healthy adult mode.

    Christof Loose shared the case of Schema Therapeutic Outpatient Treatment of a 15-year-old boy with Hypochondria Against the Background of a Car Accident Caused Paraplegia Early in Childhood.  In this case, the adolescent boy was referred for fear of dying, fatigue, depression and avoidance, along with physiological symptoms which interfered with school attendance as well as with other age-appropriate functioning. The author describes the five step schema therapy intervention that impact the teens symptoms and functioning.

    The final case discussion: “Mad for Connection” is a “Case Study in Schema Therapy for Anorexia Nervosa,” by Suzy Redston, in which schema mode work provided an understanding of the patients behaviors. Working to contain the angry child, the angry protector and the demanding child helped to find and soothe the vulnerable child, enabling the patient to work toward his goal of being able to have a healthy intimate relationship.

    Y was 12 years old when he was referred to me by his psychiatrist. "He refuses to go into the classroom for 2 weeks now," his mother told me on the phone in a low, sad voice. "We practically have to push him in, with the teacher's help, and it's not easy, he's a big boy already. And when he does get in finally, we have to go through the whole circus again the next hour."

    The reasons for the referral were vast: fear of dying, fatigue, and depression symptoms such as inactivity as well as social and emotional retreat; the teenager could not think about anything else other than his (to him unknown or awkward) physiological experiences such as dizziness or an extra systole could be the first signs of life-threatening diseases (e.g. brain tumor or heart disease). Numerous medical examinations, sometimes in emergency medical consultation, revealed no medical diagnosis. Since these problems did not only occur only at home, but also at school, significant social integration and achievement problems also arose. For example, about once or twice daily the patient had to leave the classroom, complaining about symptoms, and demanding immediate access to a rest room, where a personal school assist would help him to rest until the symptoms faded. After usually about 20-30 min, he returned to the classroom, to be met with the dismissing looks from peers and also at times from teachers.

    The following describes the case of a man referred for psychotherapy after years of various treatments for anorexia nervosa restrictive subtype both voluntary and involuntary.The use of schema mode allowed the understanding of behaviours that were life interfering and until we adopted a schema mode approach had been managed by the patient (client) being obsessional about adherence to rules in all conditions and at the expense of all else.

    The ISST Executive Board is happy to announce the call for scientific and clinical presentations and training submissions for INSPIRE 2018 in Amsterdam. This year's theme is "Corrective Emotional Experiences: How Schema Therapy Transforms Lives." We have invited top-notch keynote speakers from within and outside the schema therapy community to present their ideas, and we are inviting you to be a co-creator of the conference by submitting your proposal for workshops and other presentation formats.

    TRAINING SUBMISSIONS
    In-Congress Workshops of 180 minutes 
    These are presentations of treatment or interventions for a specific mode, clinical diagnosis or presenting problem. Workshops include demonstrations and practice opportunities with discussion of the everyday clinical use of the treatment or intervention and participants experience of the practice.

    Skill Classes of 90 minutes
    These are presentations of one intervention for a specific purpose.  Skill Sessions consist of demonstration, opportunity for participants to practice and brief discussion of their experience.
    SUBMIT YOUR TRAINING PROPOSAL HERE
    SCIENTIFIC & CLINICAL SUBMISSIONS
    Symposia
    60 (or 90)-minute presentation by 3-4 (4-5) individuals who address a common theme or topic. A minimum of 15 minutes should be reserved for questions from the audience and dialogue among presenters. The lead presenter should submit a general overview of the session. Additional individual presenters submit their abstracts within that symposium.

    Individual Papers
    Presentations on a clinical, theoretical or research topic with a typical time allocation of 10 minutes.

    Round Table
    60 minute, informal session intended to stimulate discussion on a specific topic. No formal presentations or audio/visual support are provided.

    Case Presentation
    Presenting a clinical case or a specific Schema Therapy clinical issue, followed by discussion. It can have a presenter and one or two commentators. The Chair introduces the topic, handles comments from the other participants, and fields questions from the audience.

    Poster Submission

    If you were fortunate enough to attend Summer School in Barcelona, you departed with memories that will last a lifetime. As promised, Barcelona lived up to its charm as a truly unrivaled city, ideal for a conference destination.

    We assembled a video summary of Summer School that shows glimpses of participants learning, exploring, and connecting. Barcelona's art and architecture dazzled, its cuisine delighted, its weather amazed, and its warm hospitality welcomed all. Friendships were built and renewed, and the passion driving the schema therapy community was strengthened and reinforced.

    For those who participated, thanks for the memories; and for those who were not able to join us, we invite you to browse through the photos. All can get a glimpse of what lies ahead for INSPIRE 2018 on May 24th through 26th in Amsterdam.
